There are two ways of
completing a submission form for GCLGS Services:
1. By
manually completing a hardcopy submission form.(You can
request a multi-carbon submission form by calling GCLGS
at 877 45-LABEL or print a hardcopy submission package
by CLICKING HERE)
2. By
completing a submission form on-line.
For on-line submissions,
once you have submitted your order, you will need to
print copies of the submission form as needed.
Always keep
one copy of the submission form for your records, and
send one with your submission package.
